The CYP3A4 mechanism
Grapefruit (and its juice) inhibits CYP3A4 enzymes in the gut wall, increasing the absorbed dose of medications metabolised by that enzyme. For drugs in the Eye Care and Ophthalmic Treatments class that go through CYP3A4 first-pass metabolism, regular grapefruit consumption can raise plasma levels of Latanoprost by a clinically meaningful margin and amplify side effects.

According to the prescribing information for Latanoprost, if grapefruit is flagged as an interaction, the safest practice is to avoid grapefruit entirely or to discuss timing with the prescriber. Latanoprost is a prodrug ester hydrolysed by corneal esterases to its active free acid, which selectively binds the prostaglandin F (FP) receptor. A small occasional serving may be tolerable; daily large servings around the time of Xalatan dosing are not recommended at 0.005%.
