What is it?
Wegovy is the brand name for semaglutide developed and marketed by Novo Nordisk specifically for chronic weight management. It was approved by the United States Food and Drug Administration in 2021 and by the European Medicines Agency in 2022. Wegovy uses the same active molecule as Ozempic but at a different maintenance dose and with a different therapeutic indication. It is supplied as a single-dose pre-filled pen for once-weekly subcutaneous injection.
Active ingredients
Each pen contains semaglutide as the sole active ingredient at strengths of 0.25mg, 0.5mg, 1mg, 1.7mg or 2.4mg per weekly injection. The maintenance dose for chronic weight management is typically 2.4mg per week, reached through a stepwise dose escalation. Semaglutide is a long-acting GLP-1 receptor agonist with reversible albumin binding through a fatty diacid linker, supporting weekly dosing.
Forms and dosages
Wegovy is administered once weekly by subcutaneous injection in the abdomen, thigh or upper arm. According to the prescribing information, the dose is increased every four weeks following a defined escalation schedule from 0.25mg to the 2.4mg maintenance dose, in order to limit gastrointestinal adverse effects. If a maintenance dose is not tolerated, the prescriber may consider a lower dose. Pens must be stored refrigerated before first use.
Indications
Wegovy is indicated as an adjunct to a reduced-calorie diet and increased physical activity for chronic weight management in adults with obesity (BMI 30 or higher) or with overweight (BMI 27 or higher) and at least one weight-related comorbidity such as hypertension, dyslipidaemia, type 2 diabetes or obstructive sleep apnoea. In several markets it is also approved in adolescents from 12 years with obesity. According to the prescribing information, treatment must be initiated and supervised by a clinician.
How it works
Wegovy acts on the GLP-1 receptor in the central nervous system and gastrointestinal tract to reduce appetite, slow gastric emptying and increase satiety. In the pancreas it enhances glucose-dependent insulin secretion and suppresses inappropriate glucagon release. The combined effect typically reduces caloric intake and produces clinically meaningful and progressive weight loss over months. Lifestyle measures, including a reduced-calorie diet and increased physical activity, are an essential part of the treatment plan.
Frequently asked questions
How much weight loss is expected with Wegovy? ▾
Randomised clinical trials in adults with obesity showed mean weight reduction of approximately 12% to 15% from baseline at the 2.4mg weekly dose after 68 weeks, alongside lifestyle intervention. Individual response varies and not every patient achieves these averages. According to the prescribing information, treatment continuation should be reassessed after several months if at least 5% of body weight has not been lost.
How is Wegovy different from Ozempic? ▾
Both contain the same active molecule, semaglutide, but at different maintenance doses and for different indications. Wegovy is approved for chronic weight management at a maintenance dose of 2.4mg weekly, while Ozempic is approved for type 2 diabetes at maintenance doses of 0.5 to 2mg weekly. According to the prescribing information, the two products are not interchangeable and should not be used together.
What happens if Wegovy is stopped? ▾
Discontinuation of Wegovy commonly leads to weight regain over the following months, because the appetite-suppressing and gastric-emptying effects fade as the molecule is cleared. According to clinical trials, partial regain can begin within weeks of discontinuation. For this reason, treatment is conceived as a long-term strategy, and any decision to stop should be discussed with the prescriber alongside continued lifestyle measures.
Can Wegovy be used by people with type 2 diabetes? ▾
Adults with type 2 diabetes who also have obesity or overweight with comorbidity may be candidates for Wegovy after assessment by a clinician, including review of any concomitant antidiabetic therapy. The prescribing information notes that combination with insulin or sulfonylureas may increase the risk of hypoglycaemia and that dose adjustment of these agents may be required. Wegovy and Ozempic should not be used together.
What are the main contraindications for Wegovy? ▾
Personal or family history of medullary thyroid carcinoma, multiple endocrine neoplasia type 2 and known hypersensitivity to semaglutide are formal contraindications. Caution is required in patients with previous pancreatitis, severe gastroparesis or active gallbladder disease, and during pregnancy and breastfeeding, where Wegovy is not recommended. According to the prescribing information, the medical history and current medication list must be reviewed by a clinician before any prescription.
